Government HIPAA regulations require that if you’re 18 or older, your medical records are regarded as personal and confidential and cannot be disclosed without your written consent. Edmond Dermatology Clinic HIPAA guidelines are available to you as are designation of personal representative forms. If you have questions about HIPAA, please ask a staff member to explain the details to you.
In an effort to reduce our costs (which, in turn, reduces costs for patients), we ask you to pay all appropriate charges on the day of your appointment.
We will be happy to file the necessary paperwork so your insurance company can reimburse you for charges covered by your policy. When scheduling, please inform the receptionist if your insurance requires referral or authorization from a primary care physician. If required, this referral is necessary for your insurance to cover Dr. John’s services; referrals must be received prior to your appointment.
We are a Medicare provider and will accept Medicare assignment. If you are a Medicare patient, after you have met your deductible, Medicare will pay 80% of your bill and you will be responsible for 20%. We ask that you pay your portion of these charges the day of your appointment.
